AULTMAN ORRVILLE HOSPITAL2019 ORRVILLE WALKS 5K FUN RUN 1-MILE WALK AND KIDS FUN RUN WAIVERI acknowledge and understand that I acting as participant or parentguardian am voluntarily choosing to participate in the Aultman Orrville Hospital Sponsor Orrville Walks 5K Fun Run 1-Mile Walk or Kids Fun Run individually Run or collectively Runs. I acknowledge and understand that my participation in any and all Runs whether conducted on my own coordinated by Sponsor or conducted by a third-party vendor Vertical Runner is also voluntary. I have full knowledge of and assume the risks and hazards associated with the physical effort associated with this Run. I understand that it is my sole responsibility to obtain any medical release or determination of physical fitness required to participate in the Runs and in any associated Run training activities. Risks include but are not limited to contact with other Run participants weather traffic and terrain.By signing below I along with my assigns successors heirs executors and personal representatives release Aultman Orrville Hospital and its affiliates directors officers employees agents successors and assigns from any and all claims of liability of any kind including claims for workers compensation and benefits under Ohio Revised Code Chapter 4123 and demands arising from or in any way connected to my participation in the 2019 Run including any injuries to my person including death or to my property.By signing below I waive and release Vertical Runner of Wooster Vertical Runner Race Management and all sponsors their representatives and successors from all claims of liability of any kind arising out of my participation in any Runs.I understand that there are fixed costs associated with Run registration and the Orrville Walks 5K Fun Run 1-Mile Walk or Kids Fun Run has a no refund policy. Sponsor and Vertical Runner reserve the right to cancel Runs due to unforeseen circumstances such as weather andor natural disasters.I hereby grant full permission to Sponsor and Vertical Runner the use of any photographs video tapes motion pictures recordings andor other record of Runs for any legitimate purpose.My signature below certifies that I have carefully read the foregoing that I am voluntarily accepting this waiver and that I am assuming all risks of injuries and damages that may arise while participating in the 2019 Runs.
